JOB SUMMARY
The Social Welfare Officer II shall assist in the development of Provincial/Local Inter-Agency mechanisms for the provision of interventions on cases that are beyond the resolution capacities of the City/Municipal l
inks and Local Government Unit (LGU) social workers; shall ensure the case management and caseload inventories at the Provincial level are maintained and updat
ed; perform other related tasks.
DUTIES AND RESPONSIBILITIES
Ensure that case management and caseload inventories at the provincial level are maintained and updatedAssist in the development of provincial/local inter-agency mechanisms for the provision of interventions on cases that are beyond the resolution capacities of the City/Municipal l

nducting case management processes for clients at risk, particularly those categorized as medium risk casesAssist in organizing and maintaining pool of experts at the provincial level to provide legal, psychiatric, psychological and medical interventions, if necessaryEnsure the availability of directory of services that may be appropriately tapped for case referralsAssist the focal person in co

nferences in POOs using inter-disciplinary and multi-disciplinary approachMo
