Educational Background:
• Graduate of Bachelor of Science in Nursing, Medical Technology, Radiology Technology, Physical Therapy, TESDA Certified
JOB SUMMARY:
Assist Staff Nurse by providing a defined set of patient care activities and related services necessary in caring of personal needs and comfort of the patient. Promote patient safety and support all related programs and policies to ensure delivery of excellent patient care.
Provides excellent patient care by assisting the Staff Nurse in:
a. Taking required patient’s anthropometric measurements.
b. Initiating room/facility orientation for every new admission.
c. Checking of Basic Vital Signs (Blood Pressure, Heart Rate, Respiratory Rate & Temperature) with the aid of a portable vital signs monitor and thermometer scanner.
- Standard Monitoring: every 4 hours
- For the unstable/critically ill patients or with pressor medications: AS NEEDED or per Doctor’s order
- For patients with ongoing Blood Transfusion: Every 15 minutes for the first hour then every 30 minutes for the second hour then every hour until the transfusion is finished.
- Applicable for any patients with conditions requiring monitoring (Chemotherapy, post procedure etc.)
d. Serve meals and help patients eat for those who need assistance.
e. Measuring Intake and Output (IO)
- Taking and recording of oral intake and measuring of output limited to urinary and fecal elimination.
f. Turn or reposition bedridden patients to prevent pressure sores
- Grade III to IV: hourly turning or as directed by the AMD
- Grade I to II: every 2 hours or as directed by the AMD
g. Enteral/Gastrostomy feeding
- Patency must be check first by the Bedside Nurse prior to feeding.
h. Rendering bed bath, oral care and shampoo to patients together with morning care not limited to changing of linens
i. Rendering soap suds and tap water enema.
j. Ensuring safe transfer and transport of patient during diagnostic procedures
k. Discharging patients
- Check Home Discharge Order and plan
- Orientation of discharge process
l. Reporting to Bedside Nurse/Charge Nurse for any pertinent observation on patient’s condition.
OTHERS:
1. Perform other functions as may be assigned by immediate superior from time to time
