1. Application Letter addressed to the Schools Division Superintendent.

2. Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212, Revised 2017) which can be downloaded at www.csc.gov.ph

3. Performance rating for the last three (3) rating periods.

4. Service Record; Certificate of Employment, if employed by a private company; duties and functions certified by the HR Officer/ or duly authorized official.

5. Outstanding Accomplishments.

5.1. Outstanding Employee Award.

5.2. documented Innovation(s) duly approved/concurred by the head of office.

5.3. documented research and development projects conducted.

5.4. Certificate as Resource Speaker/Trainer in seminars/training's/symposia, etc.

5.5. Articles published in newspaper/magazine of wide circulation.

6. Transcript of Records (Highest Educational Attainment).

7. Training Certificate, Certificate of Participation in seminar and/or workshops.

8. Certificate of Eligibility/ PRC ID/ Board Rating.

9. Publication which can be viewed at http://csc.gov.ph/career/

10. Omnibus Sworn Certification of the applicant stating that all documents submitted are authentic copy of the original, complete and all statements therein are true and correct. 

For Master Teacher position, the following documents must be arranged and properly labeled with do gears/side tabbing sequenced as follows: 

a. Application Letter addressed to the Schools Division Superintendent.

b. Certificate of Eligibility.

c. Personal Data Sheet (CS Form No. 212, Rev. 2017).

d. Performance rating for the last three (3) rating periods (at least Very Satisfactory).

e. Updated Service Record/Certificate of Employment.

f. Certified photocopy of Transcript of Records (Highest Educational Attainment).

g. Copy of Teachers Program (not classroom program) showing number of minutes of regular teaching load at the time the vacancy occurred.

h. Omnibus Sworn Certification, duly notarized by a Notary Public, that all documents submitted for evaluation are true and authentic copies.

i. For vacant Elementary & Junior High School item/s, additional documents indicated in Regional Memorandum No. 73, series of 1985.

j. For vacant Senior High School Item/s, additional documents indicated in DepED Order 3, Series of 2016.
